ACADEMIC ADVISING
You have been assigned an academic advisor according to the major area of study you indicated on your admission application. Your advisor is a UW-Fond du Lac faculty member or administrator who will help guide you in fulfilling your degree requirements. Your advisor will assist you in selecting your courses each semester as well as provide guidance in helping you transfer to your next college. You are encouraged to see your advisor regularly and are required to do so when you register each semester. If you change your major area of study any time after applying for admission, you need to complete a "change of advisor" form at the Student Services Office.

ACADEMIC REGULATIONS
Academic regulations are printed in the UW Colleges Catalog, which can be obtained from the Student Services Office.

ACCESS TO RECORDS
You have the right to see your personal records. A detailed statement of your rights is located in the UW Colleges Catalog. Assistance in interpreting the statement can be obtained from the Student Services Office.

ADDING COURSES
Courses which have openings may be added to your schedule during the first two weeks of the semester, or a proportionate amount of time of the summer session. You must obtain your advisor's signature before submitting the ADD form to the Student Services Office.

ADMISSIONS (New and Transfer)
Even though you are now attending classes at UW-FDL, you must make sure that the Student Services Office has a final high school or college transcript. For more information, contact the Student Services Office.

ALCOHOL POLICY
The campus will not serve alcoholic beverages at social events whenever those attending are primarily underage persons. Whenever alcohol is served, a non-alcoholic alternative must be provided. At such servings, there will be a posting of the campus' philosophy which promotes responsible use of alcohol.

APPEALS
Grade Appeals -- Students have the right to appeal grades. See UW Colleges Catalog, pg. 36, for the policy.

Appeals of Academic Regulations -- Requests for exceptions to academic regulations (e.g., return after suspension, adding or dropping courses after the deadline) can be made by completing an appropriate form at the Student Services Office. The Student Services Office personnel will forward your request to the Academic Affairs Committee.

ASSOCIATE DEGREE
You may receive the Associate of Arts and Science Degree if you fulfill the necessary requirements. Detailed information concerning these requirements can be found in the UW Colleges Catalog and at the Student Services Office. Applications for the Associate Degree should be made through the Student Services Office prior to your final semester of study. You will be notified in writing of your degree status. You may request a degree audit of your coursework at any time.

ATHLETICS
UW-Fond du Lac belongs to both the Wisconsin Collegiate Conference (13 Colleges) and the Wisconsin Junior College Athletic Conference (UW-Colleges plus technical colleges).

Fall sports include co-ed soccer, golf and women's volleyball. Winter sports include men's and women's basketball and dance team. Spring sports include club baseball and men's and women's tennis.

New freshmen are initially eligible to participate. Freshmen athletes must maintain at least a 1.5 GPA and carry at least 10 credits. Sophomores must maintain a cumulative GPA of at least 1.75 to be eligible. A minimum of six credits must be at UW-FDL for a total of 10 credits within the UW System. Any player dropping below 10 credits during the semester is immediately ineligible. One course may be by correspondence and normal progress must be shown during the semester.

All members of the athletic teams must have a medical examination and be cleared for competition by a medical doctor. A minimum of one physical examination shall be required for each student athlete for two years of competition.

AUDITING COURSES
Auditing a course is taking a course for no credit and no grade. You may wish to audit a course simply for the experience the course provides, or to avoid the penalty of a low grade for weak performance. You can change from credit to audit any time until the drop deadline.

BAND
UW-FDL and Marian College offer an opportunity for students of both colleges to play in a concert band and to receive academic credit. Some college-owned instruments are available for student use. Contact Pat Eby of the Music Department at 929-3661 for more information.

BULLETIN BOARDS/POSTERS
There is a campus policy that governs posting of flyers, posters and other materials. See Posting Policy.

BUS SERVICE
The campus is served by the Fond du Lac Area Transit. The bus stop is in the West Parking Lot. Maps and information about the routes and fares are available by calling 929-2935.

BUSINESS OFFICE
The Business Office accepts all payments for tuition, registration fees, and transcript requests. Office hours are 8:00 a.m. to 4:30 p.m. Monday through Friday. The Business Office does accept credit cards for payment of tuition. Please bring your invoice with you when you pay your fees or mail it with your check.

CAMPUS STORE

CAMPUS CLOSING
During periods of inclement weather, the Dean may find it necessary to cancel classes. Announcements to this effect will be made on the following radio stations: KFIZ, K107, WFDL, WTCX.

CAREER EXPLORATION/TRANSFER INFORMATION CENTER
The Career Exploration/Transfer Information Center in the Student Services Office can provide you with information related to careers or majors. You can learn more about your interests, values and skills through our career resources, an interest inventory and computerized assessment (SIGI).

The Transfer Information Center (TIC), contains college catalogs and other materials for all private and public colleges and universities in Wisconsin. You may browse through the TIC and take materials home with you. Additionally, college representatives routinely visit our campus. These visits are advertised on campus bulletin boards.

CATALOG
The University of Wisconsin Colleges publishes a catalog which details its vision, admission process, registration policies, academic policies and regulations, student rights and responsibilities, transfer process and course descriptions. The catalog is a vital reference for you is available in the Student Services Office.

CHANGE OF ADDRESS
You must inform the Student Services Office of any change in your name, address or other personal data. If you have a job on campus, you also need to complete a change of address card for the Payroll/Business Office.

CHORUS
A student chorus performs a wide variety of music and can be taken for academic credit. Rehearsals are at noon on Monday and Friday. Contact Pat Eby in the Music Department at 929-3661 for more information.

CLASS ATTENDANCE
Faculty members have the right to set class attendance policies for students in their classes. You will find that some instructors are more strict than others, so it's important that you understand the policy for each of your classes. No matter what your instructor's policy, you should realize that the quality of your college experience depends, in large part, on the effort you put forth. You'll get the most out of your courses and do better in them if you attend every class. It is your responsibility to make up work missed during an absence.

CLASS SCHEDULES
Separate schedules for Fall/Spring and Summer semesters are published each year. A list of classes, exam schedules, rules, regulations and other pertinent information for registering for classes is included.

COLLEGIUM
The Collegium is a formal body, consisting of faculty, academic staff and student representatives who consider academic and other matters of concern to the campus community. Meetings are held throughout the year.
Because the University of Wisconsin operates on a system of shared governance, campus committees play an important role in recommending policies to the Dean. Students hold membership on most Collegium committees, including:

Academic Affairs Committee-concerned with all aspects of academic affairs at UW-FDL (one student member).

Appointments Committee-makes recommendations to the Dean regarding faculty and staff appointments (one student member).

Budget Committee-reviews the annual budget as it is being constructed and advises the Dean on policy matters related to the budget (one student member).

Curriculum Committee-involved in the design and quality of the instructional program (one student member).

Instructional and Information Technology Committee-oversees the campus computing policy. UW System Computer Use Policy.

Steering Committee-operates for the Collegium between meetings, prepares Collegium meeting agenda, follows up on Collegium actions, nominates officers for Collegium Committee elections (one student member).

Student Life and Interests Committee (SLIC)-oversees all activities receiving financial support from Segregated University fees, reviews budgets and recommends allocation of Segregated funds to the Dean (five student members).

COMPUTER LABS
There are computer labs located in the Science Building and the Administration/Extension Building for student use. Computers provide word processing, Internet access, e-mail and other applications. Additional computers are available in the Library for student use. Labs are open from 8:00 a.m. to 9:00 p.m. Monday through Thursday, and 8:00 a.m. to 4:30 p.m. on Friday.

CREDIT BY EXAMINATION
Each institution has its own policy regarding the awarding of credit by examination. The UW Colleges Catalog details how various departments will award credit using College Level Examination Program (CLEP) tests, departmental exams or other instruments.

CREDIT LOAD POLICY
The UW Colleges Catalog states the credit load maximum policy. The maximum of 18 credits must be adhered to unless the advisor grants permission for an overload. The signed registration form constitutes the advisor's approval. Students admitted on a discretionary basis or placed on probation may be limited to 12 credits.



 D

DEAN'S OFFICE
The Dean is the chief academic and administrative officer for the campus and meets regularly with student government officers, but is available to all students. The Dean is concerned with your academic success and with your overall experience on the UW-FDL campus. If you have problems or suggestions, stop in or make an appointment with the Dean.

DEFERRED FEES
If you have received a financial aid award, but your financial aid checks have not arrived, it may be possible for you to receive a short-term deferral of fees which will allow you to avoid paying late fees. You must complete a "Request to Defer Payment of Tuition and Fees" form by the end of the second week of classes. Obtain this form at the Financial Aid Office in the Student Services Office.

DEVELOPMENTAL COURSES
If, as a result of your placement tests, you test into developmental course(s), MATH 091 and/or ENG 098, you must complete these classes during your first 30 credits at the University.

DISCIPLINARY GUIDELINES
The University has published statements which detail actions it can take to discipline students for misconduct. These guidelines are found in the UW Colleges Catalog.

DROPPING COURSES
You may drop a course during the first 10 weeks of the semester or a proportionate amount of time of a summer session. Check with your professor to determine your mid-term grade. Interaction with your advisor often helps to determine whether you should continue in the class, change to audit or drop the course. You need an advisor's signature to drop a course within the first 2 weeks of classes. After the 10th day you will need the instructor's signature.

DUAL REGISTRATION
You may have reason to register at more than one UW institution during any one term. This dual registration provision allows students to carry credits at two institutions while paying no more than the equivalent of one full-time tuition. A word of caution: you are not entitled to register at a second institution just because you have registered at the first. You must be admitted to each before you can register. Contact the Business Office for more information regarding the fee schedule.

EDUCATIONAL COUNSELING
Your academic advisor or the Student Services Office can help you in selecting your transfer institution, interpreting catalogs, contacting resources or generally clarifying your academic goals. Reference material such as catalogs from other UW institutions, the College Board Index of Majors and the College Handbook are available.

E-MAIL
Every student, faculty and staff member is assigned an electronic mailbox. Students can access their account in any of the student computing areas and off campus through an Internet browser. Free dial-in Internet access is available to students.



 F

FACULTY OFFICES
You will usually find faculty offices located in the same building where the instructors teach their classes. The faculty office corridors parallel the main corridor, on the second floor of the buildings. Faculty post their office hours and teaching room assignments on their doors. If you have difficulty meeting with an instructor, ask for an appointment time when you both can meet.

FINAL EXAM SCHEDULE
The dates and times of final examinations are generally published in the Class Schedule. However, because of changes, you should check for the latest schedule just prior to exam week. The entire revised final exam schedule is posted in the Student Services Office.

FINANCIAL AID
The Financial Aid Office, located in the Student Services Office, is here to assist you in obtaining funds to offset the cost of your education. These funds are available in the form of grants and scholarships (gift aid), loans and employment (college work-study). If you do not have sufficient funds to pay your educational costs and if you are a degree-seeking student enrolled for six or more credits, you are encouraged to apply for financial aid.

Since the main Financial Aid Office for the University of Wisconsin Colleges is located in Madison, extra time must be allowed for the applications to be processed and for communications to pass between you, our office and the Madison office. The entire process takes from two to six months. Therefore, it is very important that you apply at least by April 15 in the year preceding that in which you will need the assistance. While you may apply after this date, you should be aware that less funds are usually available and the money will most likely not arrive by the time your tuition and fees are due. To apply for financial aid, you must complete the Free Application for Federal Student Aid (FAFSA) which is available in the Financial Aid Office after January 1 each year. Even if you want to apply for just a student loan, you must complete the above listed forms in addition to a loan application. All application forms are available on the campus Web site at www.fdl.uwc.edu.

FIRE LANE WARNING
Please do not park in the fire lanes of the campus parking lots.
Vehicles parked in the fire lanes may be ticketed and/or towed at the driver's expense. Parking lot fire lanes are 20 feet wide and extend the length or width of the lot. Various markings such as ground lettering, moveable signs and permanent signs mark the fire lanes. Regardless of whether the ground markings are visible or someone has moved the portable fire lane signs, the 20 foot fire lane still exists and vehicles parked in the lanes may be ticketed and/or towed.

FIRST AID
Any need for emergency medical attention should be directed to the Business Office in the Administration/Extension building. Office personnel will then take appropriate action. First aid kits are also located in the Faculty Lounges in the Classroom and Science Buildings, the University Center and in the Athletic Director's Office in the Physical Education Building. Any accident or injury that occurs anywhere on campus must be reported within 48 hours to the Business Office.

 

Fitness Center

UW-Fond du Lac offers its students and staff access to a new, state-of-the-art Fitness Center. Located in the Physical Education Building, the Fitness Center features 15 weight lifting stations, nine cardio equipment options, free weights and two dumbbell racks, XM radio, FM radio and televisions. Locker room and towel use is included. Membership fees are $25 per year. Contact Athletic Director, Lee Watson, at 929-3663 for more information.

GRADE POINTS
The general quality of your work as a student is expressed in terms of a grade point average (GPA). Letter grades are assigned point values as follows:
A GPA is determined by multiplying the points for each course by its credit total, then adding up the points for all courses and dividing by the total credits attempted. If you do not maintain at least a 2.0 GPA, you will be placed on academic probation.

GUARANTEED TRANSFER PROGRAM
The Guaranteed Transfer Program enables you to begin your education at UW-Fond du Lac as a freshman and be guaranteed admission to a UW System institution as a junior after fulfilling certain credit and grade point average requirements. Privileges are the same as for those who begin their education at the baccalaureate institution. Forms and additional information are available in the Student Services Office.

GYMNASIUM
All students enrolled in the university are welcome to use the gym when it is not in use by classes or for other organized activities such as intramurals or athletic team practice. Your I.D. card will allow you to check out needed athletic equipment or use the weight room.

HEALTH INSURANCE
Student health insurance is not provided by the University.

HONORS AND COMMENCEMENT PROGRAM
This program is held annually in May, with faculty, administration and Associate Degree graduates in full regalia. During this ceremony, academic and organization awards are also distributed and recipients are recognized for their achievements.

HONOR SOCIETY
Alpha Nu Pi is the name of our chapter of Phi Theta Kappa, a national honor society for students at two-year colleges. Eligibility requirements are a 3.25 GPA and completion of at least 12 credits at UW-FDL. Contact the Student Services Office or the Phi Theta Kappa Office for additional information.



 I

I.D. CARDS
Your Student I.D. card is your ticket for admittance to many campus-wide activities. You must also present it whenever you check out library materials. You will be issued one identification card while you are a student at UW-Fond du Lac. I.D. photos will be taken at orientation. Replacement cost of an I.D. card is $2.

INCOMPLETES
A grade of "incomplete" is given by an instructor when a course requirement such as a paper or the final exam has not been completed. The assignment of an "incomplete" is not automatic. If you have some emergency which prevents you from completing a requirement, you must notify the instructor as soon as possible. Ask for an incomplete grade if you expect to complete the work within one semester. The instructor will submit a letter grade representing the grade that will be given if the incomplete is not satisfied on time. If the work is made up properly, the instructor will file a change of grade form,- replacing the incomplete with the earned grade.

INDEPENDENT STUDY COURSES
Several departments may offer courses in independent study. This often involves in-depth exploration of material not available through traditional courses offered at our campus. To avoid problems in the transfer of these credits to another institution, the appropriate UW Colleges department should approve the topic that the student and instructor have agreed upon.

LECTURE & FINE ARTS SERIES
UW-FDL offers lectures and fine arts programs on many interesting topics. Many are free to UW-FDL students or at a reduced admission. Notices about upcoming events are posted on campus bulletin boards.

LIBRARY

The mission of the UW-Fond du Lac Library is to support the curriculum of this institution. There are approximately 41,000 volumes in our collection. Periodicals, videos, and sound recordings supplement the book collection.

Access to these materials, as well as materials at other UW-Colleges Libraries, is provided by an Online Public Access Catalog (OPAC). The OPAC’s of all other UW System Libraries may also be searched from the same computer workstation. Through cooperative arrangements, materials not held in our library may be requested from other libraries. All UW-Fond do Lac patrons have access to over 100 online research databases, funded by a centralized UW Libraries budget.

The UW-Fond du Lac Library provides a blend of traditional and electronic service and resource to its patronage. In the library, patrons will find a quiet place to study, resources in which answers to questions may be sought and assistance in locating information.

Library Regular Semester Hours will be posted outside the Library. For current semester hours, please call 929-3616.

LOST AND FOUND
The campus' lost and found department is located in the Library in the University Center Building.



 M

MAIL SERVICE
While the campus does not have a post office, notes to faculty members or staff may be placed in the mail boxes in the mail room located in the Administration/Extension Building.

MAINTENANCE
If you need to contact the Maintenance Department for any reason, contact the Business Office before 4:30 p.m. or the Library after 4:30 p.m. A staff person will relay your request.

MEDIA LABORATORY
An audio-visual laboratory is available for you to prepare audio and visual materials for classes and student activities. Contact Tom Clausen for information (at least 24 hours in advance, if needed for a specific event).

MID-TERM PROGRESS
It is UW Colleges' policy that faculty inform students of their mid-term grades by the end of the eighth week. If you have not received your mid-term grades in your classes, contact the appropriate instructor.

MUSIC
UW-FDL students can join a student chorus, vocal jazz ensemble, and concert band. Advanced music students may also perform with the Fond du Lac Chamber Singers, Fond du Lac Chamber Orchestra or the Fond du Lac Symphonic Band. Contact Pat Eby of the Music Department at 929-3661 for more information, or come to the Music Department Office in the University Center.

OUTREACH COURSES
Continuing Education/Community Outreach is the name of the office that organizes non-credit courses for the community at large (and, under certain circumstances, credit courses). You might need to upgrade your English, math, study skills, or just wish to take a short course on a wide range of subjects. Check with the Continuing Education Office located in the Administration/Extension Building for information and a course listing.

"OWLS" Organization
"Older but Wiser Learners" is a student organization whose primary membership consists of students age 22 and older, who have had a break in their formal education.
The purpose of this organization is to encourage students to pursue their educational goals, to provide an avenue for academic and social support, to encourage a sense of belonging to the campus and a sense of identity in relation to campus life. Contact the OWLS advisor in the Student Services Office for more information.



 P

PARKING
The campus has plenty of parking and, best of all, it's free. We appreciate your courtesy by parking only between lines and not in faculty, handicapped or fire lanes. Cars not meeting these requests will be ticketed or towed at the owner's expense. To help prevent theft, lock your vehicle and cover your belongings or place them in the trunk.

PARTIAL PAYMENT PLAN
This is a great way to pay your tuition and other fees because you can spread your payments over two or three installments. There is a nominal service charge. Sign up in the Business Office before the end of the second week of classes.

PASS/FAIL GRADES
Most courses cannot be taken for a grade of pass or fail. Only elective courses which apply to the Associate's Degree or which are non-degree or LEC courses may be taken on a pass/fail basis. You may take one such course each semester. The UW Colleges Catalog has some cautions for you to consider if you think you want to enroll on a pass/fail basis.

PERSONAL COUNSELING
You probably have friends, relatives or clergy who you feel can help in times of crisis or trouble. Nevertheless, there may come a time when they are not readily available and you need help. The Student Services Office can refer you to a professional for assistance.

PHOTOCOPYING
A coin-operated photocopying machine is available in the library.

PLACEMENT TESTS
Math and English placement tests must be taken prior to registering for such classes. These tests are used to determine your skill level so that your advisor can place you in the appropriate math and English courses. Group testing is done on various dates throughout the year. Contact the Student Services Office for further information regarding the placement testing dates and locations.

POSTING POLICY
The University of Wisconsin-Fond du Lac recognizes the rights of all individuals of freedom of expression. This policy establishes procedures for the posting of all types of printed materials on the UW-Fond du Lac campus. 
Where the policy does not address a specific question, it is expected that courtesy and good judgment will determine the course of action.

General Guidelines for Public and University Postings

  • All posted materials which are not for a specific, dated event must have the posting date clearly written on the front of the material and will be removed after 30 days. Posted materials without an event date or posting date will be removed immediately.
  • No more than one poster or notice for the same event on each bulletin board.
  • Avoid placing posters on top of other posters.
  • Avoid posters larger than 11 x 17 on bulletin boards.
  • Avoid having posters extend beyond the border of the bulletin board.
  • Any material to be posted that is not in English must have an English translation posted at the same location.

Public Postings
General Use bulletin boards are available for the posting of printed materials by any member of the University community and/or the general public.


Three bulletin boards are designated for general (public) use at UW-Fond du Lac and are located:

  • In the tunnel between the Classroom and Science Buildings. -
  • In the tunnel between the Art and Administration/Extension Buildings.
  • In the University Center outside the Music Department.

All postings from outside the University community must be approved in advance in the Student Services Office, located in the Administration/Extension Building. Postings placed without prior approval will be removed.
Printed materials must conform to all applicable local, state and federal laws.

Student/University Sponsored Postings
Notices of student and other university events/activities may be posted anywhere except for specifically prohibited areas. The University group or organization that posted the materials must remove them no later than 1 day after the event.

Suggested Areas:

  • Hallway Walls.
  • Restroom Walls and Stalls.
  • Interior Doors -- but not on window areas.
  • Campus Event Bulletin Boards (Black Background with Glass Doors) located in Art, Science and Classroom Buildings
  • Campus Bookstore.
  • Library.
  • General Use Bulletin Boards. 
Prohibited Areas for All Posted Materials
  • Restricted Use Bulletin Boards -- bulletin boards clearly designated for department or other special use.
  • Chalkboards or whiteboards.
  • Exterior windows, door windows.
  • Any outside entrance door.
  • University Center Entrance Foyer.
  • Directional or informational signs and/or sign holders.
  • Stairway and balcony railings.
  • Exterior surfaces of campus buildings, light poles, directional signs, fences, benches, trees, barricades, bridges.
  • Vehicle windshields in parking lots.
  • Classrooms walls, doors and bulletin boards. 

What can be used to affix materials to surfaces?

  • Thumbtacks or other kinds of tacks can be used to secure materials to bulletin boards. Do NOT use staplers.
  • Removable plastic adhesive (U HOLD IT) can be used to attach materials in permitted areas. It is available in the
  • Business Office, Continuing Education Office, General Office, Student Services Office, Library and UW Extension.
  • Do NOT staple, nail or tack materials to walls or doors.
  • Do NOT use masking or transparent tape. 

Special Situations
Political posters/materials -- The University's commitment to freedom of speech and open inquiry requires that we be available as a forum for all discussion, including political.  However, our position is non-partisan.  If a candidate wishes to advertise here (via posters, buttons, etc.), candidates of other parties must also be allowed to do so. The only other limitation is that the campus cannot be used to raise money for political purposes. 

Commercial posters/materials -- Commercial materials may not be displayed on campus unless they are offering services or employment to students. 

Dissemination of Posting Policy
Copies of this policy will be given to every campus organization and kept in the Library, the Student Government Office, General Office, Business Office, Student Services Office and Maintenance Office.
Student Government and other student organizations will explain the posting policy at the beginning of each year at the first general meeting. 

This policy will be included in the Student Handbook and in orientation materials for new faculty and staff.

PROBATIONARY STANDING
Students are placed on Academic Probation when their grade point average (GPA) drops below 2.0. Probationary status is a warning that you should take appropriate action to improve your achievement. Be sure to discuss your options with your advisor since failure to make academic improvements may result in suspension from the College.



 R

RE-ENTERING STUDENTS
If you have not had continuous enrollment from one semester to another (excluding summer), you must file a new admission application and submit official transcripts from any college or university you have attended since the last enrollment date at a UW College.

If you were dropped or suspended from school, you must re-apply for admission. If re-admitted, your admission status will be that of final probation.

REFUNDS
Refunds of tuition can be received through the Business Office. The catalog and timetable both contain the schedule for refunds. Students withdrawing from school or dropping and adding courses must complete the proper paperwork at the Student Services Office.

REGISTRATION
You may register for classes in the Student Services Office, from 8:00 a.m. to 4:00 p.m. weekdays. You must meet with your advisor prior to registration. Register early to ensure you get the classes you want.

REPEATING COURSES
You may repeat a course as often as you wish. The last grade earned will be calculated in you GPA. If you decide to repeat a course, be sure to mark the space on the registration form or else the original grade will still be averaged in with your other grades.

ROOM RESERVATIONS
You or your group may reserve a room on campus for student activities. To reserve a room, complete a "Facility Use Reservation" form which is available in Mail Room, Administration/Extension Building Rm-129.

SCHOLARSHIPS
Scholarship applications are available in the Student Services Office. The University of Wisconsin-Fond du Lac Foundation annually awards new student scholarships, continuing student scholarships and adult student scholarships. Additional information about the scholarships can be obtained from the Student Services Office or online.

SEXUAL HARASSMENT
Sexual harassment by students, faculty or staff will not be tolerated by this University. If you experience such behavior, report it immediately to the Dean or the Student Services Director for action. The policy regarding sexual harassment is available in the library, from the Dean's Office, from the Student Services Office and in your Student Information Guide.

SPECIAL NEEDS SERVICES
UW-FDL is committed to providing equal educational opportunity and access for all students. Students with special needs are offered a variety of services to ensure that programs and facilities are accessible.
Students with special needs are asked to confer with the Student Services Director and their instructor, as early as possible, so that individualized accommodations can be arranged.

STUDENT ACTIVITIES COMMITTEE
This group plans activities for students to learn and socialize on campus. Students work with the Student Activities Coordinator to bring in a variety of entertainers and speakers for students. In the past, students have seen hypnotists, magicians, singers, speakers, played games, had dances and sponsored trips to other events outside the campus.

STUDENT ASSISTANCE PROGRAM
If you need help coping with alcohol and/or drug misuse, there are several individuals and programs, including referral, readily available to you. Contact the Student Services Director for information. The assistance you receive will be kept confidential.

STUDENT EMPLOYMENT
There are a number of employment opportunities available for you. Apply for any student job at UW-FDL in the Student Services Office.

College Work-Study (CWS):
If you have applied for financial aid and have been awarded college work-study in your financial aid award, you may apply for any student job on campus and will be given first priority over students who have not received college work-study.

Student Assistant (SA):
If you are not eligible for college work-study, you may apply for positions which are funded with institutional funds in the Student Services Office.

Off Campus:
Employers contact UW-FDL looking for students to work in their organizations. A loose-leaf notebook with these opportunities is located in the Student Services Office.

STUDENT GOVERNMENT
The purpose of Student Government is to promote the general welfare and interest of the student body and the College, and to provide representatives to Collegium committees (see "Collegium"). Contact the Student Services Office or the Student Government Office if you are interested in becoming a member of Student Government.

STUDENT SERVICES OFFICE
The Student Services Office provides you with a wide range of services including admissions, financial aid, registration, veteran's affairs, advising, testing and unofficial transcripts. Call 920-929-3606 for more information.

STUDY DAYS
Each semester, special days called "Study Days" are set aside for the purpose of preparing for exams. No student activities are to be scheduled for this time unless the Collegium gives prior approval. This special arrangement indicates the high level of importance faculty place on exam preparation.

STUDY SKILLS
The University expects that you will display good study habits. If you feel that you are in need of developing better study skills, the Student Services Office can help you to obtain aids and referral resources.

SUMMER SESSION
The summer session courses are usually taught over an eight-week period. The intensity and speed with which the material is covered is approximately twice the semester rate. The average full-time load is six credits.

SUSPENSION
If you perform at a level below the accepted GPA, you could be suspended for one semester or more. You may appeal your suspension to the Academic Appeals Committee. At the completion of your suspension, you must submit an application to the Student Services Office requesting re-admission.

TELEPHONES
A pay phone is available in the Physical Education Building. Campus phones in staff offices are not to be used for personal calls.

TORNADO SHELTERS
In the event of severe weather conditions, you will be asked to take cover in a designated shelter. One is located in each building. Classroom and Science Buildings x hallway in the lower level of each building. Administration/Extension, University Center – kitchen. Physical Education Building – gymnasium. Physical Plant – lower level storeroom.

TRANSCRIPTS
To request a transcript, complete the form at the Business Office and pay the required fee of $3 per transcript. Your request will be forwarded to the UW Colleges Registrar in Madison. If you are not able to fill out the form, simply mail your written request and the fee to UWC Registrar, P. O. Box 8680, Madison, WI 53708-8680. Be sure to include your original signature on the request, social security number, name changes which might have occurred, the address of the recipient, and the approximate dates of attendance. The check should be made payable to: UW Colleges.
To inform the transfer institution of your final course grades, it might be necessary to send another transcript after the final grades have been posted. This can be done with the initial request by adding another three dollars and asking for the mailing or the transcript at the time of application and after grade posting.

TUITION AND FEES
Tuition and fees are paid at the Business Office. A partial payment plan is also available. In addition to the tuition payment set by the state, you also pay a Segregated Fee and United Council Fee each semester which supports the athletic department, dances, guest speakers, various cultural programs and United Council.

UNIVERSITY CENTER
The University Center offers activities and services for students and faculty at UW-FDL. This facility contains the cafeteria, theater, art gallery, TV area, bookstore, library, student commons and meeting rooms. The University Center is a place where you can relax between classes, socialize with your classmates, watch TV, get something to eat or attend one of the programs sponsored by student groups.

UW-EXTENSION CORRESPONDENCE COURSES
If you are a full-time student and want to take a UW EX correspondence course (one not
offered on our campus), and if you apply during the first two weeks of the semester, you will be allowed to enroll in the course for a minimal fee and the cost of the textbook. Forms are available in the UW-Extension Office.

UW COLLEGES SENATE
The Senate is a recommending body of faculty, students, academic staff, the Deans and the central administrative office. Their shared governance recommends policy and standards for the UW Colleges.

VENDING MACHINES
Vending machines are located in the University Center and in the Physical Education Building. Money lost in a vending machine should be reported to the Business Office.

VETERANS' BENEFITS
If you are seeking veterans' benefits, obtain the necessary forms from the local VA Office. We will report your registration and attendance history to the VA. The Student Services Office will not be able to process your VA papers until you first obtain your certificate of eligibility.

WELLNESS ALLIANCE
The Wellness Alliance is a group of students who organize activities which promote healthy lifestyles and heighten student awareness of the effects of drug and alcohol abuse. For more information on becoming a member of this organization, check with the Student Services Coordinator.

WITHDRAWAL POLICY
If you wish to end your relationship with the University before the end of a semester, you may do so within the first 10 weeks of a semester or a proportionate amount of time of summer school. You must fill out the appropriate form for your withdrawal to be official (don't assume that your record will be clear if you simply stop attending).

WITHHOLDING OF INFORMATION
You have the right to request that certain information, such as your address and phone number, not be given out to the public and not be printed in the student directory. If you feel the need to exercise this right, contact the Student Services Office.

Work Zone
The "Zone" offers academic support services to UW-Fond du Lac students and promotes active, cooperative learning in a comfortable atmosphere. Free tutoring, study skills counseling and peer mentoring are offered. The "Zone" is a very supportive place dedicated to helping students succeed on both a personal and an academic level. Located in the Classroom Building, second floor.
